Dare to Care – Coordinated, comprehensive social services and
care management
Our professional care managers help keep you and your loved ones safe and
independent by providing these services:
- assess individual needs
- connect to community resources
- identify available benefits and entitlements
- coordinate services to ensure your well-being
In crisis?
Our care managers are available 24 hours a day, seven days a week. We work to
quickly restore safety and reduce anxiety. We make home visits and maintain
telephone contact so you are not alone. We deal with emergencies as they arise:
- immediate crisis
- hospitalization
- relocation to a secure environment
- dangerous or at-risk situations
- self-neglect
- domestic abuse
- financial exploitation
Out of control?
Sometimes the little things seem to pile up into an unmanageable mess. Our care
managers:
- help restore order
- review your mail
- make sure bills are paid
- organize your appointments
- coordinate activities
o transportation
o shopping
o medical appointments
- communicate with others
o health and medical providers
o professional service providers
o long-distance family members
- provide supportive counseling and guidance
We ensure that you’ll receive services as well as dignified and compassionate
treatment.
