CareSource is a not-for-profit corporation.  We provide services regardless of the person’s ability to pay.  Our work is funded through program services fees, grants, and donations. 

However, the number of persons served is directly related to the current level of our funding available.  In order to use our sliding scale to determine the program service fee, the client and his or her representative must complete a verified financial affidavit.

The rates listed below are for individuals who do not meet (or choose not to participate in) our financial intake process.

Flat Fee Packages

Initial Assessment.... $750.00
Includes an initial telephone consultation with one of CareSource's Directors, an onsite personal assessment of the individual, an environmental safety assessment and any required follow-up to verify the information gathered at the assessment. We will then give the requesting party a detailed written report of our findings and make our recommendations as to what steps should be taken next.

In Office Consultation.... $175.00
This includes a one-hour consultation with one of our Directors. The initial 15 - 30 minutes is comprised of an intake to determine the client's needs. The remaining time is spent laying out a plan of action, including specific contacts, agencies, entitlements, etc.

Medicaid Applications.... $750.00
Includes one or more meetings (usually in the home) with the client to assist with the application and prepare paperwork for the actual interview; attendance to the Medicaid interview and any follow-up tasks necessary to complete the application process (up to 6 hours.)

The Economy Plan
Flat monthly fee based on a set number of visits with defined parameters for any necessary follow-up tasks. Client must be compliant with services (memory impairment may preclude this payment option.) Fee to be determined during initial assessment phase.

Daily Money Management or Medical Claims Processing

Initial Consultation.... $100.00
The initial consultation is with a Director or the Financial Administrator.  During this time we will review the client's needs, status of all bills, accounts and incomes and develop a plan of action, as well as determine the location of ongoing services.

Hourly Rates

Hourly rates begin upon the conclusion of an Initial Assessment or Consultation (see above.) Time is billed in 6-minute increments (1/10th of an hour.)
