CareSource has offered a global array of services for the disabled, frail and elderly, including Care Management, Professional Guardianship and Daily Money Management since 1987. Because each client's need for our services varies, we will complete up to a 30-minute telephone assessment to understand the current situation. Then, if requested, we will visit the home or institution and make a complete assessment of the individual's personal, medical, social and financial needs.

CareSource prefers for both a responsible party and the client to sign a contract for services that defines the Plan of Care. Care plans are goals agreed upon by the client, the responsible party (family or fiduciary) and CareSource. These objectives often include both short and long-term goals, as well as emergency planning (such as where to go in case of a hurricane.)

CareSource, Inc. can:

• Evaluate the client's needs
• Write a plan of care
• Set up systematic controls and programs
• Implement the recommended plan of care and systems

We can also stop at any one of these several steps, turning the remaining project over to another involved party.

Should CareSource remain involved, we will see that the care plan is implemented and is working, adjusting and correcting the plan as necessary through home visits, advocacy, ongoing observation and supervision.
